The Licensed Practical Nurse (LPN) CMA, provides direct staff support to the Community Care Program in planning, designing, integrating, implementing, modifying and evaluating the effectiveness of program component and/or services to improve the quality of patient care and the appropriateness and timeliness of services provided.

  • Total Rewards of a Allied Health Professional The LPN-CMA is responsible for providing the clinical guidance and expertise for the coordination of Veteran's Community Care. The LPN-CMA works in collaboration with the RN Care Manager and Advanced Medical Support Assistants in care coordination of Veteran's outpatient and inpatient community care by determining eligibility and payment methodology. The LPN-CMA verifies care delivery, requests additional clinical information and collaborates with VA providers and community providers/vendors in ensuring the Veteran receives access to quality community care. Duties include but are not limited to: Receives electronic notifications of consults and/or community hospitalized notifications by accessing alerts in CPRS. Verifies Veteran's eligibility status utilizing eligibility tool and determines Community care payment methodology. Verifies justification for referral, reason for request, care required, medical justification, type-frequency-estimated length of services, etc. Verifies use of appropriate consult and forwards if necessary to appropriate service. Confirms provision of community outpatient and/inpatient care. Requests clinical information/documentation to confirm care or to extend episode of care. Notifies RN Care Manager and documents on consults of no-shows, cancellations, or refusals of care appointments. Runs daily pending, active and scheduled reports to ensure consults are acted on timely, confirm care and request clarification as needed. Updates, monitors, and tracks all authorized Community Care services in the FBCS system and VISTA Appointment Management System. Works collaboratively with the RN Care Manager in coordination of care for outpatient and inpatient episodes of care. Establishes, maintains and exercises an effective resource data base of contacts within the VA system and in the community to assist in facilitation and coordination of Veteran's care within the VA system and in the community. As needed, will work with the IFC/IFT RN Care Manager in facilitation of Veteran's inpatient and outpatient care. Contacts community providers, vendors and/or VA vendors/providers for additional clinical orders, documentation, etc. as needed to facilitate Veteran's care. Refer urgent/stat alerts to RN Care Manager and/or providers for timely care coordination. Works with Advanced Medical Support Assistants (AMSAs) in ensuring care is authorized appropriately. Work Schedule: Monday - Friday 7:30am - 4:00pm or 8:00am - 4:30pm; subject to change based on the needs of the facility Recruitment/Relocation Incentives (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases. Education

Establishes constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills. Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure. Serves as a preceptor in orienting, educating, and training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures.

The actual grade at which an applicant may be selected for this vacancy is in the range of GS-3 to GS-6. Physical Requirements: Physical aspects associated with work required of this assignment are typical for the occupation, see Duties section for essential job duties of the position. May require standing, lifting, carrying, sitting, stooping, bending, puling, and pushing. May be required to wear personal protective equipment and undergo annual TB screening or testing as conditions of employment.

Work Environment: Work is performed in an office/clinic setting with minimal risks that requires normal safety precautions; the area is adequately lighted, heated, and ventilated. However, the work environment requires someone with the ability to handle several tasks at once in sometimes stressful situations. Note: Only education or degrees recognized by the U.S. Department of Education from accredited colleges, universities, schools, or institutions may be used to qualify for Federal employment.

Requirements
